The following immunization would be the
MINIMAL number for a completed immunization status
for a student in grades 1 - 12:

At least 4 DPT Boosters if the student is under the age of 15 with the last
dose received on or after the 4th birthday; if 15 or
older, the student must have had a Tetanum-diphtheria
(TD) booster (due 10 years after last DPT).

3 doses of Polio vaccine
with last dose received on or after the 4th birthday. |

2 MMR with the first given
on or after first birthday, and second after the age
of 15 months OR laboratory evidence of immunity to
measles, mumps and rubella.

Hepatitis B vaccination
is required for all new school entrants beginning January
1, 2000.

| |
Varicella (Chicken Pox)
or present documented immunity from a reliable source.
This could be a parent’s statement that their
child had previously had varicella disease
. |

Clinics are held every Thursday
at the Tuscola Health Department in Caro, excluding
holidays. Hours are 9:00 a.m. - 11:15 a.m. and 12:45
p.m. - 3:00 p.m. the first Thursday of the month. Clinic
hours are from 1:00 p.m. - 6:00 p.m. both walk-in and
scheduled appointments are available.

Clinics serve children 2-3
months through school age. A parent or legal guardian
must read and sign our information forms prior to receiving
immunizations. IF A PARENT IS NOT PRESENT WITH
THE CHILD, A SIGNED CONSENT FORM MUST BE COMPLETED.
OTHERWISE
IMMUNIZATIONS WILL NOT BE GIVEN. Children should be
in good health when presented for immunization. For

Just a reminder, if your child must stay
in from recess or be taken out of gym class, a note,
signed by your doctor, is required. If your child is
well enough to be in school, he/she should be well
enough to go out to recess or participate in gym class. |
