Please bring :
1. child’s original birth certificate
2. immunization record
3. social security number
4. two proof of residency, e.g. utility
bill, driver’ s license, etc.
Health, transportation, and emergency
forms will be given to each parent to be completed at that
time.
If your child will be five years old on
or before December 1, 2007 , or if you know someone whose
child is eligible, please attend spring registration.
